Three Steps to Athletic Eligibility

1. Complete Registration on FinalForms - Register your student through FinalForms. The link can be found on the CMS website under Athletics/Activities.

2. Pay Athletic Fees and ASB Card Fees - Choose one of the following payment options:

In Person: The CMS office is open Monday–Friday, 7:15 AM–3:30 PM.
Online: Visit the CMS website and click on Payment Portal under the Students & Families tab.

3. Submit a Sports Physical - Turn in a hard copy of your student’s sports physical to the CMS office before the first day of practice.
